MEDICAL SCRIBING. Medical scribing is the process of documenting patient encounters in real-time in an Electronic Health Record (EHR). The scribe captures the physician-patient interaction, including the patient history, physical exam, assessments, and plans. The scribe may also help with basic clerical tasks such as scheduling, filing, and retrieving medical records, obtaining lab results, and managing patient flow. Most medical scribes are certified medical assistants or have completed a medical scribe training program.
